In the original online early release version of this article, the Introduction contained an error in a quote attributed to C.M. Dobson. The word “developing” was inserted for “developed.” The html, print, and PDF versions appear correctly. For reference, the corrected sentence follows.

It may well be the most “costly and disruptive disease in the developed world” (Dobson, 2003), and thus investigation of its causes and possible cures is of some interest to us all.
